At 365 Hospice, we’re dedicated to providing the highest level of service in everything we do. From the moment patients return home, our team delivers customized hospice care everyday for our patients with life-limiting and terminal illnesses and their loved ones. We know that this is a difficult time for everyone and the services our team offers are here to help at every step of the way. We serve patients with life-limiting and terminal illnesses, including heart disease, Multiple Sclerosis, cancer, stroke, lung and pulmonary disease, liver disease, kidney disease, ALS and Alzheimer’s. Please read on below to learn more about our services.

Physician Service

Helping to provide best possible quality of life is the goal of our professional hospice physicians. Our doctors are at the core of our customized hospice care teams and work with other members of the team to develop the care plan that best meets the individual needs of each patient and their loved ones. Our hospice physicians closely coordinate with the patient’s primary care physician to ensure care is delivered according to the patient’s needs and individual wishes. All of our hospice physician services are provided by an individually licensed practitioner under state law, authorized to practice medicine or osteopathy. to practice medicine or osteopathy.

Personal Care

At 365 Hospice, we care for the whole person and are ready to assist in many ways beyond medical treatment. Our staff can help with tasks like dressing, grooming, oral hygiene, denture care, shaving, bathing and anything else that delivers comfort, dignity and quality of life. We’ll also go further for you – doing the little things that make a big difference, from a trip to the store, making sure supplies are on hand, helping out around the house and many other ways we can help when we’re needed.

Respite Care

Even the strongest caregiver can struggle with the demands of caring for a loved one, day after day. At 365 Hospice, we can arrange for a period of respite care – a short-term accommodation in a medical or hospice facility outside the home in which a loved one may be placed. This provides temporary relief – or respite – to those who are caring for family members, who might otherwise require permanent placement in a facility outside the home. This period of respite care can help caregivers relax and tend to other important matters.

Companionship

365 Hospice companions provide emotional support and a shoulder to lean on. Our hospice companions are caring, passionate people who understand just how important and rewarding a friend can be in caring for the whole person. Oftentimes, while the patient is in need of care, the family is also struggling with emotional decisions necessary to ensure a comfortable end-of-life for their loved one. Our companions provide a helpful “ear,” and work to make sure family members also receive the attention they need to take one day at a time.
